JCHC Providers (Johns Creek House Call Providers) perform the highest quality healthcare to people in their homes, office or place or work environment. We focus on treatment of acute and chronic conditions. JCHC Providers can serve as your regular health care provider.

Among many services, we conduct routine tests, in your home, such as blood work, urinalyses, EKGs and x-rays. We also diagnose and treat infections, minor laceration repairs, abscesses; and chronic conditions such as diabetes, high blood pressure and COPD. We’re also proud to offer IV hydration!

We’re also able to refill/prescribe medications and may refer to other specialists as needed. If you feel you’re having a life threatening emergency, call 911 or go to the nearest emergency room.

Meet our Provider

Evelyn Adio, RN, MSN –FNP, AACNP.

Evelyn graduated with honors from the Chamberlain College of Nursing, Master of Science Program specializing as a Family Nurse Practitioner. She obtained a Bachelor of Science (Nursing) from Jacksonville University School of Nursing in Florida and her Associate of Science Degree in Nursing from Century College/Inver Hills Community College in Minnesota. She started her career working as a cardiac nurse at Allina Healthcare in St. Paul Minnesota before relocating to Georgia’s Emory Healthcare System where she worked with a wide variety of patient populations since 2006. Evelyn is a Board Certified Family Nurse Practitioner and a member of the Honor Society of Nursing, Sigma Theta Tau International. She is also a Member of American Academy of Nurse Practitioners. She believes in back to the basics care and a patient-centered approach to medicine. She emphasizes on patient education in each encounter and is strong on preventive medicine.
