Welcome to Johns Creek House Call Providers (JCHC Providers) Serving the North Atlanta area (Johns Creek, Cummings, Lawrenceville, Roswell and Alpharetta areas).

→ There are people in our community with no primary care providers

    Johns Creek House Call Providers aim at providing the highest quality and most convenient and patient-centered healthcare to people in their homes, office or place or work environment. We focus on health promotion and maintenance through the diagnosis and treatment of acute and chronic conditions. We provide some of the same care provided by physicians and maintain close working relationships with our collaborating physician. JCHC Providers can serve as a patient’s regular health care provider. Among many services, we conduct routine tests in the patient’s home, such as blood work, urinalyses, EKGs and x-rays. We also diagnose and treat acute conditions such as infections and minor laceration repairs, abscesses; and chronic conditions such as diabetes, high blood pressure and COPD. We are able to refill/prescribe medications and may refer to other specialists as needed. We do not treat life threatening emergencies. Call 911 to go to the nearest emergency room. Providers also do not walk around with narcotics..

    Pricing

    → $100 per house call, (That include assessment, diagnoses and treatment.)
    → $70 Sports Physical
    → $25 for each additional test or procedure
    → The first visit is 100 dollars and subsequent visits are 75 dollars

    Fees are payable at the time of visit by credit card, cheque or cash.

    Can’t get out of the house, or feel too sick to drive?
    → Want more privacy and personalized attention?
    → Want to avoid exposure to other sick people?
    → Are you having difficulty using public or private transportation because of your health/mobility limitations?
    → Avoid long waits in the Emergency rooms. Our mobile practice provides similar services as office based clinics in the comfort of your homes.

    A typical house call may involve a first time visit to a new patient’s home from a nurse practitioner who pulls up in a car with a laptop computer and little black bag filled with portable medical equipment and supplies. The nurse practitioner conducts a comprehensive assessment that may last up to an hour depending on a patient’s health condition with or without help from their support system. A plan of care is then design that may include treatment and follow up visit if needed.
    Do you provide breathing treatments
    Yes, if a patient needs a breathing treatment we will provide treatments at your home.

    No, at this time we only accept ‘Self pay patients’

    What payment type do you accept?
    We accept all major credit cards and cash payments.

    Appointments can be scheduled within a day or two, and in some cases, the same day for problems that are urgent, but not life threatening

    It is important to note that JCHC Providers LLC do not provide true emergency services and are not a replacement for calling 911
